Preparing a notice of collection

A Notice of Collection is used to obtain individuals' consent for the university to collect, use, and disclose personal information.  There is specific information that a Notice must include such as:

  • The legal authority the university has to collect the information;

  • The purpose(s) for which the information is being collected;

  • Contact information in the event of questions or concerns. 

University departments considering the collection of personal information (either paper or electronic) should contact the Policy and Compliance Advisor regarding the development of a Notice of Collection.

Standard Notice of Collection

The following is a standard Notice of Collection which may, and should be, included on any documents or web forms whose purpose is to collect personal information.

Personal information on this form is collected under the authority of the University of Ontario Institute of Technology Act, SO 2002, c. 8, Sch. O. and will be collected, protected, used, disclosed and retained in compliance with Ontario’s Freedom of Information and Protection of Privacy Act R.S.O. 1990, c. F.31.
